Instructions: How to download the applications.

1. Choose the format you like from one of the .HTML, .DOC and .PDF formats.
2. For .PDF you need to have Acrobat Reader, If you don't have one, Please download it here.
3. For .Doc you need Microsoft word plugin, if you use Netscape Browser
4. Do not change any of the contents in the .doc files.
5. Use your browser File/ Print option to print it.
6. Please make sure the application(s) were printed properly.
